Mashlim from your Kupat Holim.

Sold by Clalit (Mushlam Zahav, Mushlam Platinum), Maccabi (Maccabi Sheli, Maccabi Zahav), Meuhedet (Adi, Si), and Leumit (Zahav). Costs NIS 30 to NIS 100 per month per adult. Covers private surgeon choice, second opinion, expanded specialists, dental basics, and some alternative medicine.

Bituach Bri'ut Prati (full private policy).

Sold by Harel, Phoenix, Menora Mivtachim, Clal, Migdal, and AIG Israel. Costs NIS 80 to NIS 400 per month per adult. Adds catastrophic illness coverage (cancer, transplant, stroke), overseas medical care, expensive medications outside the Sal Bri'ut, and full private hospital stays.

Most well-prepared Israelis carry both layers.

Who should buy what

Healthy adults under 50 with kids. Both layers. Mashlim from the Kupah plus a basic private policy. Total cost: NIS 150 to NIS 300 per month for the family.

Single adults under 35. Mashlim only, possibly with a smaller catastrophic-only private policy. NIS 50 to NIS 120 per month.

Olim with chronic conditions. Mashlim is almost always worth it (no underwriting). A private policy is harder to get and will likely exclude the existing condition, but other catastrophic risks (a new cancer, accident, surgery) are still covered.

Seniors 65+. Premiums rise sharply. Lock in early or accept higher cost. Some insurers stop accepting new policies past 70.

Brand by brand

Harel Mehulelet

The market leader. Strongest brand for surgery coverage. Pays out for private surgeons in Israel and abroad, including elective procedures. Mid-to-high premium tier. Good claims service. English-speaking adjusters available.

Phoenix Royal

Aggressive on price for under-45s. Strong dental add-on. Slightly tougher claims process. Good for tech employees because many employers offer Phoenix as the group plan.

Menora Mivtachim

Mid-range. Solid for families with children. Best add-on for sports injuries (popular with athletes and active Olim).

Clal Bri'ut

The largest in life and health combined. Strong overseas coverage. Slightly higher premiums.

Migdal Bri'ut

Conservative pricing. Long history of paying difficult claims. Slower to approve experimental treatments.

AIG Israel

Strong on travel medical and short-term coverage. Lighter on long-term family policies.

What the policy actually covers

A typical NIS 150 per month policy for a 35 year old adult covers:

- Surgery (Nituchim). Private surgeon choice in Israel for up to NIS 500,000 per procedure. You pay a small deductible (Hishtatfut Atzmit), typically NIS 1,500.

- Catastrophic illness (Mahalot Kashot). Lump-sum payout of NIS 200,000 to NIS 1,000,000 if diagnosed with cancer, heart attack, stroke, or similar major illness.

- Overseas care (Tipulim BeChul). Up to NIS 1,500,000 for treatment abroad if Israeli care is unavailable or unsuitable. Includes flights for the patient and one companion.

- Medications outside Sal Bri'ut. Reimburses for drugs the Kupah will not cover, including some cancer immunotherapies and rare-disease treatments.

- Transplants. Full cost coverage including overseas if needed.

What it does not cover

- Routine dental for adults (buy a dental rider separately)

- Cosmetic surgery

- Pre-existing chronic conditions (unless explicitly added with surcharge)

- Pregnancy complications (covered, but not routine prenatal care, which is in Mashlim)

- Eye surgery for vision correction (LASIK is in Mashlim Platinum tier)

Pricing examples (2026)

For a non-smoker, no major pre-existing conditions, mid-range coverage:

- Single adult age 30: NIS 95 to NIS 140 per month

- Single adult age 45: NIS 150 to NIS 220 per month

- Couple ages 35 and 33: NIS 220 to NIS 320 per month combined

- Family of 4 (parents 35, kids 3 and 7): NIS 290 to NIS 420 per month total

- Single adult age 65: NIS 350 to NIS 550 per month

When to buy

Within the first 6 months of aliyah if possible. Insurers underwrite based on age and health on the day you apply. Premiums rise 4 to 8 percent per year of age, and any chronic condition discovered later may lead to exclusions if added at renewal.

Buy through a broker (Sochen Bituach). Brokers are paid by the insurer and the service is free. They handle the medical disclosure forms, negotiate exclusions, and run side-by-side quotes from 4 to 6 insurers in one meeting.

Common mistakes Olim make

Buying the cheapest policy without reading the limits. Some policies cap surgery at NIS 100,000 per claim, which barely covers a routine knee replacement.

Skipping the medical disclosure form. Failing to disclose a known condition voids the policy. Always disclose, even minor things.

Buying through the bank. Banks sell insurance with a commission markup. Always cross-check with a broker.

Waiting until you need it. Once you have a diagnosis, premiums rise sharply and key conditions get excluded.

What to do this month

1. Activate Mashlim from your Kupat Holim if you have not already.

2. Get 3 quotes from a broker for a private policy. Match the coverage levels side by side, not just the premium.
